No show Policy and Cancellation of an Appointment
Please be courteous and call promptly if you are unable to attend your appointment, at least 24 hours notice is required to cancel or reschedule appointments.
Annual well exam with your primary care provider is required and ALL medication management appointments must be kept in order to have medication refilled.
**IMPORTANT: Due to the high demand of these appointments; Two CANCELED, RESCHEDULED or NO-SHOW appointments per family will result in no further behavioral health appointments being scheduled.
