NEW PATIENTS: Welcome to our practice, our team looks forward to meeting you and your child.  We are here to help you navigate the transition of your child's medical care.

To register please complete the new patient packet and return to our office.

You may email your completed packet to:  medrecords@carteretclinic.com, fax it to 252-726-7441 or drop it off at our front desk. 

As soon as our onboarding coordinator receives your new patient packet, they will contact you to complete your child’s registration and request your child’s medical records from their previous provider.   We understand that choosing your child’s medical provider is a very important decision and we thank you for choosing Carteret Children’s Clinic as your child’s medical home.  Please feel free to contact us with any questions.

NEWBORNS: Congratulations on your new little one.  We understand that choosing your baby’s medical provider is one of the many important decisions you will make for them.  Many parents choose to meet the medical provider and office staff before deciding where they will feel comfortable taking their baby for care. At this visit, you will have the opportunity to meet one of our providers and have your questions answered.
